How do galaxies central black hole form?

I have read that black holes are essential to galaxy formation, at least for spiral galaxies. But typically a galaxy is much more than 100,000 times as massive than the central black hole. Will most mass concentration nucleus for a proto-galaxy evolve directly into a black hole? Or do the central stars appear first, then collapse / merge into a black hole?
